What is Care Planning?

A care plan is a comprehensive program that entails all of the care needs of you or your senior. The plan focuses on personal needs such as help at home, home health, physical therapy, occupational therapy, even home safety evaluations.

Each of these plans is tailored to the individual.  PIECES believes in Person Centered Care, which means we are not just looking at the physical needs of the person. Taking into account that person wishes, their dignity and their ideal quality of life creates the perfect plan that will promote a positive outlook on their life.

This is a plan for the future. Life is unpredictable and has many pitfalls and unexpected events. At PIECES, we aim to help you make a plan for those events so that you don’t have to panic when they happen.

Your Individualized Roadmap

The information you share with us helps us develop a “Roadmap” for your future care. Each roadmap details events that might occur in your life and a plan of how to handle that situation if it comes up. 

This includes:

  1. A recap of your current plan/path called “The Current Route”
  2. Possible events that will occur called “Expected Exits”
  3. A plan for the worst events called “The Emergency Ramp”
  4. How to use each plan

We are never done!

As we said before, life is unpredictable. Sometimes things can happen to you or your family that no one could ever predict or plan for. Should something happen in your life that conflicts with our initial plan, call us!
